 DESCRIBE THE EXACT PROBLEM

What EXACT steps were taken to cause the error/problem?
Are you able to duplicate this error/problem every time (even after a
full reboot)? If not every time, how often does the error occur?
In the text box below, please cut and paste in your system information
using the following instructions:
Click Start in the Windows taskbar.
Select Programs.
Select TPA Software.
Select TPA System Information Compiler.
When the program opens click File and select Copy To Clipboard.
When prompted to include Installed Applications information, click Yes.
Click Exit.
Finally, place the mouse cursor in the text box below, right-click on your mouse and select Paste.
